Care Coordination

JFS’ Care Coordination Program is a free service that helps streamline care and improve the health and well-being of individuals and families. Working with you, a dedicated care coordinator will also work with your providers and your entire support system.

Eligibility requirements do apply, including active enrollment in Medicaid and having two or more chronic health conditions such as, asthma, diabetes, heart disease, mental health condition, or substance use disorder. Alternatively, those with a diagnosed serious mental health illness, who are living with HIV/AIDS, or sickle cell disease qualify for the program.

Social Care Navigation (1115 Waiver Programming)

Our Social Care Navigation program helps individuals enrolled in Medicaid explore and enroll in additional social support services. If you are eligible for this program, you will be matched with a navigator who will connect you with health-related social services including housing support (additional eligibility requirements apply) and social care management. If you have additional needs, your navigator can help connect you with additional services at JFS or other services in the community.

What’s Social Care Navigation?

In a social care navigation system, you sit at the center of a group of people looking to support your well-being. Your navigator acts as a mediator between you and outside services, guiding you through application, enrollment, and ongoing care processes. JFS’ navigators help you find and secure the social support services you need to thrive, working with you to meet your unique needs. Services they might help connect you with include:

  • Nutrition
  • Housing Assistance (additional eligibility requirements apply)
  • Mental Health Care
  • Social Care

Housing Navigation Program

JFS’ Housing Navigation program will help you find, secure, and maintain housing. We offer:

  • Pre-Tenancy Services to help you prepare for your move-in
  • Housing Navigation – Locating temporary housing or a rental
  • Community Transitional Supports – Housing support when you are leaving a hospital or other institutional setting
  • Utility Setup – We’ll help you work with utility companies as you move
  • Tenancy Sustaining Services – Your navigator can assist with issues that arise after move-in to make sure you can
