What’s Critical Time Intervention?
Critical Time Intervention, or CTI provides time limited, tiered support to clients facing mental health challenges. An expert team including a nurse, Peer Mentors, and a dedicated Case Manager work together with each client to help them find stability during critical times of transition such as hospital discharge.
We partner with Erie County Medical Center (ECMC) to provide a landing place for patients leaving the Emergency Room or an inpatient psychiatric stay. After enrolling in the program, clients work with their CTI team who offers transitional support, charts pathways to recovery based on clients’ personal goals, and reconnects clients with community supports that hold meaning to them such as support groups, family and friends, or religious organizations. JFS has the unique ability to refer clients to other services within our organization, creating the potential for seamless client care and support.
